This week HDC celebrated the first day of SUMMER with a summer solstice celebration!

Day Habilitation

Developing Potential Day By Day

HDC offers community day habilitation services as well as facility-based day habilitation services. This program is available in both Frederick and Washington counties for individuals that require assistance and training with meaningful day activities designed to promote independence and personal choice of daily living, academic skills training, community mobility training, acquiring proper work habits, and pre-vocational skills training. Community volunteer activities are available, and each center provides an array of in-center and community activities to enhance sensory experiences, increased socialization skills, and community participation.

HDC facility-based services have changed over the past year with participant choice as the driving principle. The participant chooses which services they would like to receive. While still providing a safe and supervised environment, participants are free to choose their daily schedules, choosing from a variety of options within the center and the community.

Services are available 5 days a week, up to 8 hours per day, Monday through Friday.

Transportation

Transportation by a HDC fleet vehicle is included in many services we offer.  HDC operates several forms of accessible vans, SUVs, and buses.  Agency transportation includes transport to and from the centers, homes, community activities, and job sites. Each of our residential homes also has its own vehicle for transportation.

Nursing Services

HDC has a full nursing department staffed with all Maryland Registered Nurses.

Nursing Support Services are available for each HDC individual, these services may include case management and/or delegation services to certified CMT staff who have successfully completed the MTTP class.  24-hour-on-call nursing services are available to all staff.

HDC Coordinator

Everyone is assigned a HDC case manager to assist with planning, monitoring, and achieving their health and/or human services goals based on each service HDC provides.
