CDS

Community Development Services (CDS) is a community-based door-to-door service HDC offers as an alternative to a facility-based, Day Habilitation service.  

CDS operates in staff to participant ratios of 1:1 to 1:4. By providing an array of community experiences and learning opportunities, individuals receiving CDS support will not only benefit from increased social and recreational opportunities, but also will ultimately help create a pathway to employment by improving the individual’s employability through community participation and the development of vocational interests that is based on a person’s interests, skills, and abilities.

Community Development Services offers community experiences that include volunteer activities, travel training by using public transportation and community resources, enrolling in classes, workshops, and seminars, participating in community recreational and social activities, peer counseling, discussion and activity planning groups, and health and wellness counseling.

Transportation

Transportation by a HDC fleet vehicle is included in many services we offer.  HDC operates several forms of accessible vans, SUVs, and buses.  Agency transportation includes transport to and from the centers, homes, community activities, and job sites. Each of our residential homes also has its own vehicle for transportation. 

Nursing Services

HDC has a full nursing department staffed with all Maryland Registered Nurses.   

Nursing Support Services are available for each HDC individual, these services may include case management and/or delegation services to certified CMT staff who have successfully completed the MTTP class.  24-hour-on-call nursing services are available to all staff. 

HDC Coordinator

Everyone is assigned a HDC case manager to assist with planning, monitoring, and achieving their health and/or human services goals based on each service HDC provides.

Where do I begin?

HDC is a provider of Developmental Disabilities Administration (DDA) Services. People choosing HDC services must already have an approved wavier status from DDA.  

Go to:  https://health.maryland.gov/dda/Pages/Home.aspx to apply first, then reach out to HDC once the wavier is approved.

Typically 40-60 business days depending on the type of services.  

The Human Development Council is a non-profit agency that receives funds from State and Federal Governments. There is no out of pockets expenses for families and/or individuals that have been approved for services.
